Insurance

I am an in-network provider for:

• Ambetter
• Anthem BCBS
• Medicare
• Optum
• United Healthcare
• UMR
• United Behavioral Health

     If you participate in an insurance that I am not in network with, and you have out of network benefits, I will be happy to provide a Superbill for you to submit for reimbursement within your plan’s limits.

Private Pay

     $150 per session for private pay. I do save some spots for sliding scale clients so please let me know if you need help.

Payment

     Cash, check, and major credit cards are accepted for payment.

Cancellation Policy

     If you are a no-show for your scheduled appointment, or you do not give at least a 24-hour notice of a cancellation, you will be charged the full session fee. I do realize that life happens, so I will offer a once yearly free pass that you can apply to your session fee.
